<p style="text-align: justify;"><span style="font-size: 12pt;">Our client, based in Derbyshire, is seeking an experienced Adult Social Worker to join their Prevention and Personalisation Team. </span><span style="font-size: 12pt;">This role focuses on&nbsp;early intervention, strengths-based practice, and promoting independence, supporting adults to live well within their communities. You will deliver&nbsp;personalised, outcome-focused support&nbsp;in line with the Care Act 2014, working with a range of needs while effectively managing risk.</span><br><br><span style="font-size: 12pt;"><strong>Key Responsibilities<br></strong></span><br><span style="font-size: 12pt;">Core duties include:</span></p> <ul style="text-align: justify;"> <li style="font-size: 12pt;"><span style="font-size: 12pt;">Managing a caseload of adults, focusing on&nbsp;prevention, early intervention, and personalised support planning.</span></li> <li style="font-size: 12pt;"><span style="font-size: 12pt;">Undertaking&nbsp;Care Act 2014 assessments, strengths-based reviews, and outcome-focused support plans.</span></li> <li style="font-size: 12pt;"><span style="font-size: 12pt;">Promoting independence through&nbsp;reablement, assistive technology, and community-based solutions.</span></li> <li style="font-size: 12pt;"><span style="font-size: 12pt;">Supporting individuals to access&nbsp;local community resources, voluntary sector services, and informal support networks.</span></li> <li style="font-size: 12pt;"><span style="font-size: 12pt;">Carrying out proportionate&nbsp;risk assessments and risk management, balancing empowerment with safeguarding.</span></li> <li style="font-size: 12pt;"><span style="font-size: 12pt;">Completing&nbsp;Mental Capacity Assessments&nbsp;and contributing to&nbsp;Best Interests decision-making&nbsp;where required.</span></li> <li style="font-size: 12pt;"><span style="font-size: 12pt;">Working within key legislative frameworks including:</span> <ul> <li style="font-size: 12pt;"><span style="font-size: 12pt;">Care Act 2014</span></li> <li style="font-size: 12pt;"><span style="font-size: 12pt;">Mental Capacity Act 2005 (MCA)</span></li> <li style="font-size: 12pt;"><span style="font-size: 12pt;">Human Rights Act 1998 (HRA)</span></li> </ul> </li> <li style="font-size: 12pt;"><span style="font-size: 12pt;">Leading and contributing to&nbsp;Safeguarding Adults processes, including Section 42 enquiries where required.</span></li> <li style="font-size: 12pt;"><span style="font-size: 12pt;">Applying a&nbsp;strengths-based and person-centred approach&nbsp;to all interventions.</span></li> <li style="font-size: 12pt;"><span style="font-size: 12pt;">Working collaboratively across&nbsp;multi-disciplinary teams (MDT), including health professionals, housing, and voluntary organisations.</span></li> <li style="font-size: 12pt;"><span style="font-size: 12pt;">Supporting&nbsp;hospital discharges and prevention of admission&nbsp;where appropriate.</span></li> <li style="font-size: 12pt;"><span style="font-size: 12pt;">Maintaining&nbsp;accurate, timely, and high-quality case records&nbsp;in line with statutory requirements.</span></li> <li style="font-size: 12pt;"><span style="font-size: 12pt;">Demonstrating professional curiosity, sound judgement, and&nbsp;evidence-based decision-making.</span></li> </ul> <p style="text-align: justify;"><span style="font-size: 12pt;"><strong>Qualifications &amp;
