<div>Liquid Personnel is currently recruiting for an experienced Hospital Social Worker to join our client's busy Hospital Discharge Team in Bury. This is an excellent opportunity for a skilled practitioner with previous hospital social work experience to support adults with complex needs and play a key role in facilitating safe, effective, and timely hospital discharges.</div>
<p><strong>What will your responsibilities be?</strong></p>
<ul>
<li>Manage a hospital-based social work caseload.</li>
<li>Complete assessments and support planning to facilitate safe discharges.</li>
<li>Work closely with NHS professionals, patients, families, and partner agencies.</li>
<li>Coordinate care packages and community support to prevent delayed discharges.</li>
<li>Undertake safeguarding responsibilities and risk assessments where required.</li>
<li>Maintain accurate and timely case records.</li>
<li>Provide cross-site cover across multiple hospital locations as required.</li>
<li>Participate in weekend working on a rota basis.</li>
</ul>
<p><strong>Qualifications & Experience:</strong></p>
<p>To be successful in this role you must have, </p>
<ul>
<li>Qualified Social Worker (BA, MA, DipSW, CQSW or equivalent).</li>
<li>Registered with Social Work England.</li>
<li>Previous hospital social work experience is essential.</li>
<li>Experience supporting hospital discharge planning.</li>
<li>Strong assessment, communication, and multi-agency working skills.</li>
<li>Ability to work flexibly across hospital sites and weekends.</li>
</ul>
