<li>Undertake strengths‑based assessments in line with relevant legislation</li>
<li>Manage a varied caseload across the Chichester and Bognor area</li>
<li>Develop and implement person‑centred support plans aimed at improving wellbeing</li>
<li>Identify eligible needs, personalised outcomes, and appropriate interventions</li>
<li>Work collaboratively with individuals, families, carers, and community assets</li>
<li>Use clinical reasoning to recommend adaptations, equipment, and reablement pathways</li>
<li>Contribute to a supportive learning environment with regular professional supervision</li>
</ul>

<li>HCPC‑registered Occupational Therapist</li>
<li>Strong understanding of the Care Act and strengths‑based practice</li>
<li>Experience completing functional assessments and developing outcome‑focused plans</li>
<li>Confident managing a caseload within a supportive MDT environment</li>
<li>Ability to work independently while contributing to team‑wide goals</li>
