<p style="text-align: justify;"><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;">Our client based in Herefordshire is seeking an experienced Social Worker to join their Community Wellbeing and Social Work Team (CAAsT). This is an excellent opportunity for a motivated practitioner to work within a strengths-based, person-centred service, supporting adults with a wide range of needs, including older people, individuals with physical disabilities, learning disabilities, and mental health needs.</span></p>
<p style="text-align: justify;"><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;">The successful candidate will manage a varied community-based caseload, undertaking Care Act Assessments, Mental Capacity Assessments, safeguarding enquiries, Continuing Healthcare (CHC) work, Section 117 responsibilities, reviews, and support planning. You will play a key role in promoting independence, wellbeing, and positive outcomes whilst working collaboratively with individuals, carers, families, health professionals, and partner agencies.</span></p>

<li style="font-family: arial, helvetica, sans-serif; font-size: 12pt;"><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;">Developing and reviewing person-centred care and support plans that promote independence, wellbeing, choice, and community inclusion.</span></li>
<li style="font-family: arial, helvetica, sans-serif; font-size: 12pt;"><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;">Undertaking Mental Capacity Assessments and applying the principles of the Mental Capacity Act 2005 to support informed decision-making.</span></li>
<li style="font-family: arial, helvetica, sans-serif; font-size: 12pt;"><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;">Completing safeguarding enquiries, including Section 42 enquiries, ensuring adults are protected from abuse, neglect, self-neglect, and exploitation.</span></li>
<li style="font-family: arial, helvetica, sans-serif; font-size: 12pt;"><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;">Assessing and managing risk, implementing appropriate safeguarding measures whilst promoting least restrictive practice.</span></li>
<li style="font-family: arial, helvetica, sans-serif; font-size: 12pt;"><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;">Working with adults with physical disabilities, learning disabilities, mental health needs, and older adults, providing tailored interventions to meet individual needs.</span></li>
<li style="font-family: arial, helvetica, sans-serif; font-size: 12pt;"><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;">Undertaking Continuing Healthcare (CHC) assessments and contributing to Decision Support Tools (DSTs) where required.</span></li>
<li style="font-family: arial, helvetica, sans-serif; font-size: 12pt;"><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;">Completing Section 117 aftercare work, supporting individuals transitioning from secondary mental health services and ensuring appropriate support arrangements are in place.</span></li>

<li style="font-family: arial, helvetica, sans-serif; font-size: 12pt;"><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;">Supporting individuals through periods of crisis and increased vulnerability, utilising professional judgement and available resources to achieve positive outcomes.</span></li>
<li style="font-family: arial, helvetica, sans-serif; font-size: 12pt;"><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;">Coordinating services and resources with health providers, community organisations, and commissioned services to meet assessed needs.</span></li>

<ul style="text-align: justify;">
<li style="font-family: arial, helvetica, sans-serif; font-size: 12pt;"><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;">Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, or CQSW.</span></li>
<li style="font-family: arial, helvetica, sans-serif; font-size: 12pt;"><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;">Registration: Must be registered with Social Work England.</span></li>
<li style="font-family: arial, helvetica, sans-serif; font-size: 12pt;"><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;">Experience: Significant post-qualified experience within Adult Social Care, preferably within a community-based setting.</span></li>
<li style="font-family: arial, helvetica, sans-serif; font-size: 12pt;"><span style="font-family: arial, helvetica, sans-serif; font-size: 12pt;">Legislation Knowledge: Strong understanding of the Care Act 2014, Mental Capacity Act 2005, Adult Safeguarding legislation, Continuing Healthcare Framework, Section 117 responsibilities, and associated statutory guidance.</span></li>
