We are seeking a compassionate, motivated Full-Time Physical Therapist (PT) to provide skilled home health therapy services throughout the Southeast Michigan area , including communities in Oakland, Macomb, Wayne and Washtenaw Counties.

💙 What You’ll Do
As a Home Health Physical Therapist, you’ll work directly with patients in their homes to improve strength, mobility, balance, safety, and overall functional independence.
You will:
• Complete comprehensive physical therapy evaluations and identify functional limitations and rehabilitation needs.
• Develop individualized plans of care, measurable goals, and appropriate treatment strategies.
• Provide skilled therapeutic exercise, strengthening, gait training, balance training, transfer training, and functional mobility interventions.
• Assess fall risk, home safety, and appropriate use of assistive devices.
• Educate patients and caregivers on safety, fall prevention, mobility techniques, and Home Exercise Programs (HEPs).
• Monitor patient response and progress throughout the episode of care.
• Complete reassessments and modify treatment plans when clinically appropriate.
• Coordinate care with physicians, caregivers, nursing, therapy disciplines, and other members of the interdisciplinary team.
• Maintain accurate, timely, and compliant clinical documentation.
• Provide services consistent with physician orders, the established plan of care, and applicable Medicare/CMS requirements.
💼 What We’re Looking For
Our ideal Physical Therapist has:
• Degree from an accredited Physical Therapy program.
• Current Michigan Physical Therapist license in good standing.
• Strong clinical assessment, critical-thinking, and problem-solving skills.
• Excellent patient and caregiver education skills.
• Ability to work independently in the field while collaborating effectively with the interdisciplinary team.
• Reliable transportation, a valid driver’s license, and current automobile insurance.
• Home health experience preferred.
• Familiarity with Medicare/CMS home health documentation requirements preferred.
