As a Physical Therapist at KeystoneCare, you will make a meaningful impact by helping patients strengthen mobility, restore function, and maintain independence through compassionate in-home care. This role is ideal for someone who is energized by one-on-one patient support and collaborative care, and who wants to contribute to better outcomes for individuals and families across the communities we serve. Responsibilities • Provide personalized rehabilitation services to patients in their homes. • Assess, plan, and carry out treatment programs focused on improving mobility, strength, and overall functional abilities. • Collaborate closely with a multidisciplinary team to support patient care and recovery. • Guide patients through their recovery journey with compassion and professionalism. • Help restore patients' independence and well-being through high-quality therapeutic care. • Graduate of an accredited Physical Therapy program • Current Pennsylvania Physical Therapy License • Minimum two years of physical therapy experience in Home Health or similar settings • Experience treating geriatric patients in a home health or similar care setting • Experience with post-surgical rehabilitation, neurological conditions, and orthopedic rehabilitation in a home health or similar care setting • Experience providing care in hospice-adjacent contexts • Valid Driver’s License and reliable vehicle • Proof of Auto Insurance • Strong communication skills for effective collaboration with patients, families, and the care team • Excellent organizational abilities and experience managing a diverse caseload • Eligible to work in the United States • Experience with Matrix Care is a plus. Experience Physical Therapy: 2 years (Required) License/Certification: PA Physical Therapy License (Required)
