Director of Health Services - RN (DHS) Type: Full-Time Shift/Schedule: Day shift Location: Washington Gardens Memory Care, 9000 SW 91st Avenue, Tigard, Oregon 97223 Apply at  www.TeamAvamere.com As a Director of Health Services in an Assisted Living Facility, Resident Care Facility, or Memory Care Facility, you will lead and manage the health and well-being of residents, ensuring compliance, coordinating care, and providing leadership and training to the health services team. As the Director of Health Services, you will play a key role in providing leadership and mentorship to care staff, ensuring that the care we provide aligns with state regulations and organizational policies. The ideal candidate will possess excellent clinical skills, strong leadership abilities, and a passion for working with seniors in a long-term care setting. Essential Duties and Job Responsibilities Resident Assessments : Conduct thorough assessments of new and existing residents to identify healthcare needs, preferences, and goals. Continuously update care plans based on changes in health status. Care Planning & Documentation : Develop, implement, and evaluate individualized, person-centered care plans in collaboration with residents, families, and interdisciplinary team members. Maintain accurate and up-to-date nursing documentation in compliance with state regulations and facility policies. Regulatory Compliance : Ensure the facility adheres to all state and federal regulations, healthcare standards, and best practices. Participate in audits and quality assurance reviews to maintain high levels of compliance. Medication Management : Oversee the Health Service Department, completing audits of medication administration for compliance with state and federal regulations. Assist staff with medication management processes, ensuring the safe and accurate delivery of prescribed treatments and medications. Staff Leadership & Education : Provide direction, mentorship, and support to medication and care staff. Assist with training on nursing protocols, medication administration, and emergency procedures. Promote a culture of teamwork, accountability and communication. Collaboration & Communication : Foster open communication with residents, families, staff, and outside providers to ensure coordinated, collaborative care for residents in our community. Participate in interdisciplinary team (IDT) meetings and family care conferences to discuss residents’ progress and concerns. Healthcare Coordination : Coordinate with outside providers, including physicians, therapists, and hospitals, to ensure continuity of care and address any special healthcare needs. Problem-Solving & Critical Thinking : Handle dynamic and evolving healthcare situations by applying critical thinking skills and effectively responding to emergencies both in person and remotely, addressing changes in resident health, and staff challenges. Quality Improvement : Participate in continuous quality improvement initiatives aimed at enhancing care delivery, resident satisfaction, and staff engagement. Assist in monitoring key performance indicators and identifying areas for improvement. On Call requirements: Participate in on-call rotation for after-hours, weekends and emergencies. Other duties assigned are appropriate to the position. • Must possess a nursing degree from an accredited college or university. • Must possess a current, unencumbered, active license to practice as an RN in this state. BSN preferred. • Nursing experience in assisted living, Alzheimer/dementia, long term care, geriatric rehab, sub-acute and/or critical care clinical setting. • Thorough knowledge of CMS and State Regulations regarding patient care in an ALF/IL/MC facility. • Experience in electronic medical records documentation. • Strong leadership and communication skills. • Ability to design and deliver effective training programs. • Knowledge of State and Federal regulations and compliance requirements. • Excellent organizational and time management skills. • Must have and maintain an active CPR/BLS during employment. • Maintain confidentiality of all resident care information. • A genuine desire to improve the quality of life for seniors. • Create and uphold an atmosphere of warmth, patience, enthusiasm, and a calm and cheerful environment.
