Estelle's Professional Community Services is seeking a dedicated and compassionate Qualified Mental Health Professional (QMHP) to join our Assertive Community Treatment (ACT) program team. Responsibilities
• Conduct comprehensive mental health assessments to determine client needs and eligibility for services.
• Develop and implement individualized treatment plans in collaboration with clients and interdisciplinary team members.
• Provide crisis intervention and support to clients in community settings to ensure safety and stability.
• Collaborate with psychiatrists, nurses, social workers, and other team members to coordinate care.
• Monitor client progress and adjust treatment plans as necessary to achieve desired outcomes.
• Facilitate client access to community resources such as housing, employment, and healthcare services.
• Maintain detailed documentation and comply with all regulatory and agency standards.
• Hold a valid QMHP certification or equivalent licensure as required by state regulations.
• Minimum of a bachelor’s degree in psychology, social work, counseling, or a related field.
• More than 1 year of experience working with individuals diagnosed with serious mental illnesses within community-based programs is a plus.
• Strong knowledge of ACT model principles and evidence-based mental health practices.
• Experience engaging clients and interdisciplinary team members in a mental health care setting.
• Ability to work independently and manage multiple mental health cases with flexibility and professionalism.
• Must be eligible to work in the United States.
• Willingness to provide services in community settings, including client homes and other locations as needed.
